Below is a list of agencies where you may obtain applications, regulations and other important information along with some common payroll forms:

Important Payroll Dates


By the 15th of each month - if you are a monthly filer

Deposit income, social security and medicare taxes for the previous month. Use Form 8109 coupon and make the payment at your bank or use EFTPS. You may pay these taxes with Form 941 if your total tax liability for the quarter is less than $2,500 and the taxes are paid in full with a timely filed return. (See Circular E to determine if you are a monthly filer).

By January 31

Furnish Forms 1099-MISC - Miscellaneous income & W-2 - Wage and tax statement

File Form 940 or 940-EZ - Employer's Annual Federal Unemployment Tax (FUTA) tax return

By February 28

File Forms 1099 with Form 1096 with the IRS

File Forms W-2 with W-3 with the Social Security Administration (SSA)

By April 30, July 31, October 31 and January 31

File Form 941 - Employer's Quarterly Federal Tax Return and deposit any underdeposited income, social security and medicare taxes. You may pay these taxes with Form 941 if your total tax liability for the quarter is less than $2,500 and the taxes are paid in full with a timely filed return.

Deposit Federal Unemployment (FUTA) tax due if it is more than $500. Use Form 8109 coupon and make payment at your bank or use EFTPS.

File Form NUCS-4072 - Employer's Quarterly Contribution and Wage Report with the Nevada Department of Employment Security

File Nevada Modified Business Tax Return with the Nevada Department of Taxation

By December 31

Order or purchase forms W-2, 1099-MISC, W-3 and 1096

Note: If any due date falls on a Saturday, Sunday, or Federal holiday, use the next business day. A statewide legal holiday delays a filing due date only if the IRS office where you are required to file is located in that state.
